Led by a highly experienced practice nurse and supported by our full GP team. We provide structured, ongoing care for conditions including type 2 diabetes, asthma, COPD, heart disease, atrial fibrillation and stroke — with regular scheduled reviews covering blood tests, blood pressure, medication review and personalised care planning, so problems are caught early and your condition stays well controlled. Eligible medical card and GP visit card patients can avail of this care free of charge under the HSE Chronic Disease Management Programme.
