Illness Benefit & Medical Certificates
If you are unable to work due to illness, you may be entitled to Illness Benefit from the Department of Social Protection. To apply for Illness Benefit, you will need a medical certificate, known as a Certificate of Incapacity for Work, from your GP. Your GP can complete and submit this certificate electronically through a secure online system. Depending on your illness, your GP may be able to provide a certificate covering the full period of your absence. In some cases, you may need to attend a further consultation for review and an extension of your certificate.
Applying for Illness Benefit
You can open your Illness Benefit claim online here through the Department of Social Protection. Please note that your employer may also require a separate medical certificate (sick note/work certificate) to confirm your absence from work. Your employer should advise you of their requirements, including how many days you can be absent before a medical certificate is required.
Extending a Medical Certificate
You must be assessed by a GP before an Illness Benefit certificate or medical certificate can be issued. If you already have a certificate and need to request an extension, you can complete the medical certificate extension form below. We will review your request and contact you if you need to attend the practice for a further GP assessment.
Please do not use the form to request an initial certificate — you will need to arrange a GP consultation for this.
